Bring cats in a secure carrier and dogs on a secure leash. Note their name, species, age or birth date, and anything that has changed recently.
Current details
Bring medication names, dosage directions, known allergies, current food or diet details, and any questions you want to remember in the room.
Previous context
Tell the clinic the previous hospital’s name and location. Call with questions about records or what to bring.
